I would like to use autologin on my NT 4.0 clients however I also need to be
able to logout of the default username and login as a local admin. The issue
I have is if I try to "Close all programs and log in as a different user" the
system just logs in again on the default account. Is there another key I am
missing?

I believe this article http://support.microsoft.com/?id=114615 -
Bypassing Automatic Logon in Windows NT will answer your question.
