When writing or replying to email, the settings in options such as to always
send in plain text and to indent replies with a caret ">" are ignored. Any
clues?

In Tools | Options | Send, is: Reply to messages using the format in which
they were sent checked? That would explain the switch to HTML.
Is the Indent with > always ignored? If sometime, the above could be a cause
since HTML doesn't use the > for indentation.
Also, if the message was sent in Plain Text, but using Quoted Printable, OE
will indent the Indent On Reply.

You're probably replying to a Quoted/Printable formatted message, Neil. No
workaround for that (other than OE-QuoteFix).

I think you're right. This is a new/reinstall of a system that crashed that
had OE-QuoteFix, but no more. I may try to dig it up again.
