I need glucose and blood pressure daily record

I need to keep track of glucose and blood pressure once a day. I also need a
little space for possible additional remarks. I'd like it in the form of a
12 month, 1 page calendar. I'm not very computer adept. Can you help me?

Look through the templates at
http://office.microsoft.com/en-us/templates/results.aspx?qu=calendar&av=TPL000
and find one you like (and that is usable in whatever Office program/version
you have).
