I have created a "locked" form to be used and cannot send it and keep it as a
"Read Only." So I need to include a "Clear Form" button in form. Is there
such a thing? Or is there a way to send the document so that it retains its
"read only" status?

Instead of clearing the contents of the form, set up a form *template* which
can be used repeatedly (to create a particular type of forms). This simply
means that you should save your form as a template document (*.dot in Word
97-2003; *.dotx or *.dotm in Word 2007). For more, see
