I created a form using IPM.nots. I am trying to send the form to recipients
so they can reply, fill it out, and send it back to me. However, when they
reply, it created a new message (without the form). If they forward the form
back to me, it does send the form back to me but when they fill out the form,
it comes back blank on my end.

Thanks Sue,
I've unchecked the box, now when I send it, the recipients don't even see
the form at all. All they see is a regulare e-mail message.

Did you read the article I suggested? It explains how both sender and
recipient need access to the published form.
